At HARMONY AND FOCUS, we respect the privacy of every visitor to our SITES, including this website, our MOBILE app, our PATIENT PORTAL & our scheduling & billing site. This Privacy Policy explains how we collect, use, disclose, and protect personal information provided to us via the Sites by patients and our online visitors. Please review this Privacy Policy, as it contains information, terms, and conditions that may affect your legal rights and ability to use the SITES. We know you care about your personal information, and we want you to be fully informed about our practices. In addition, we encourage you to become familiar with the privacy policies of our vendors which may include additional disclosures and policies separate than ours.
Please note that this Privacy Policy only applies to personal information you provide through your use of the Sites and may be shared with our vendors in order for us to provicde you efficient care. This Privacy Policy does not apply to any Protected Health Information collected via the SITES. Rather, our Notice of Privacy Practices will govern our use and disclosure of your Protected Health Information. For more information, including information about your rights under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (collectively, “HIPAA”), please review our Notice of Privacy Practices document during on-boarding with our practice.
By accessing the Sites or using any of the features or services provided on the Sites, you consent to the information collection and use practices described herein. If you do not consent or otherwise do not agree with our Privacy Policy, please do not use or access the Sites.
PERSONAL INFORMATION WE COLLECT
HARMONY AND FOCUS may collect certain information from you through the Sites, including personal information (i.e., individual information that may be used to personally identify or contact you). We may collect this information actively (i.e., by collecting information you voluntarily submit to us) and passively (i.e., by collecting information automatically gathered from your computer).
ACTIVE COLLECTION
At several places on the Sites, HARMONY AND FOCUS may collect certain personal information you voluntarily provide. For example, when you create an account, schedule an appointment, fill out a form, or join an email distribution list, we or one of our vendors may collect your name, address, phone number, and/or email. If a vendor collects your personal information, such vendor may share all or part of your personal information with us.
You may also use the Sites to pay a bill, buy products or make a charitable donation to HARMONY AND FOCUS (collectively, “Transactions”). To process a Transaction, HARMONY AND FOCUS or one of our vendors will collect personal information you, such as your name, billing address, credit or debit card number, email address, telephone number, and email address. Our vendors or we will process you credit card payments using established encryption techniques for protecting your information.
If a vendor processes your Transaction, we encourage you to read such vendor's privacy policy and terms and conditions of use before entering any personal information. HARMONY AND FOCUS may not receive complete payment information for a vendor-processed Transaction, but our vendors will provide us with some personal information upon completion of the Transaction, such as your name, billing address, email address, telephone number, amount paid or donated, how you heard about HARMONY AND FOCUS, and the last four digits of your credit card number, the latter of which we use for verification and/or customer service purposes.
You may have the option to register for receipt of email communications, such as electronic newsletters, that we disseminate from time to time. Registration for such communications may include providing us with your name, email address, and potentially demographic information. If you send us an email request or participate in our forums, we may store that information as well, including any personal information included in a post, message, or email, as well as any personal information included in any attachments you may send include in such post, message, or email.
PASSIVE COLLECTION
When accessing, browsing, or using the Sites, our software may automatically collect certain information from your device (e.g., computer, smartphone, and tablet) using “cookies” and other tracking techniques, or by collecting platform information from our servers.

- Direct primary care (DPC) is a healthcare model where patients pay a membership fee to receive comprehensive primary care services directly from a physician or a group of physicians, without involving insurance companies. This approach allows patients to receive longer appointments, better access to their healthcare provider, and more personalized care. In a DPC clinic, patients typically pay a monthly or annual fee, which covers all or most of the primary care services they may need, such as office visits, check-ups, preventative care, chronic disease management, and basic laboratory tests. This fee also allows patients to communicate with their physician via phone, email, or video chat, which can eliminate the need for unnecessary in-person visits. Compared to a traditional fee-for-service model, where insurance companies reimburse healthcare providers for each service provided, DPC clinics offer several advantages to patients. For example, DPC clinics may have fewer administrative costs, which can translate into lower healthcare costs overall. Additionally, the absence of insurance billing and coding can allow physicians to spend more time with their patients and focus on providing high-quality care rather than meeting billing requirements. - The initial consultation and membership costs $1999/year and covers 5 follow-up visits. During this time we will refine your personalized wellness plan and monitor your progress. Additional visits cost $300/hr. A monthly membership fee is also an option. Any Laboratory and screening testing is an additional fee. However, we try to keep to costs reasonable. We have partnered with area labs and have negotiated rates on your behalf.
- Generally speaking, if the client has not yet committed to initial consultation and by contacting our office to request a cancellation a refund may be issued, however after the initial consultation the fee is not refundable. Also, if a client schedules an initial consultation and fails to show up (a NO SHOW) then the consultation fee is non-refundable.
- Yes, gifting our services would be a lovely gesture. Please bare in mind that you should be permitted to share your loved one's contact information as part of our SITES policy (see Privacy above) in order to coordinate our services.

- No, we have elected NOT to participate in Medicare.
- While still offering everything a typical primary care clinic offers, Harmony and Focus specializes in health and wellness rather that disease. As such, we primarily keep you healthy and are attempting to fill a necessary void in the healthcare system by keeping you well. You should keep your affiliation with a traditional primary care physician as well as health insurance coverage for catastrophic requirements.
- Yes. Harmony and Focus Wellness Clinic does not take the place of general health insurance coverage or catastrophic coverage. Our practice is a focused on health and wellness and as such is not a typical primary care practice, We do not participate in a health insurance program. You are advised to continue your HSA, PPO, Medicare or other insurance programs.
- We have elected not to participate as an in-network provider for any commercial health insurance plans at this time ( however, this may change in the future). As an out-of-network provider, diagnostic tests that we order may be submitted to your insurance company for reimbursement, however, out of pocket expenses are required to be paid at time of service.
- It is unlikely that this service will be reimbursable. However, some Flexible Spending Account and Health Savings Account plans may pay for all or part of the annual fee. Clients are advised to consult their human resources representative at their place of employment, or their FSA or HSA plan managers.
- The fee is a medical expense and may be deductible. Patients are advised to consult with their tax consultant to clarify qualification in their particular circumstance.
- If you have a life threatening emergency, call 911 immediately. After you call 911, please keep us informed.
- At present, we do not have admitting privileges a local hospitals, however we will work in collaboration with your hospitalist should you require hospitalization.

- Patients are free to see any specialist they wish. We are available to help you decide which specialists to see and to coordinate such consultations.
